This is a sad story. This is why accidents happen on mines and people die: Ten silly little things happen; each is trivial and insignificant in and of itself; but they all align in just the right way and then there are one, two, three and four people dead.
The blame lies with every one of the ten people who let each of the ten silly little things happen. Even though each of them is as innocent of ill intent as new snow. And not one of them is guilty of classic negligence.
